A case of hepatocellular carcinoma mimicking cavernous hemangioma on Tc-99m RBC liver SPECT

Abstract

Few reports of false-positive Tc-99m RBC scintigraphy have suggested a cavernous hemangioma in patients with hepatocellular carcinoma. A 68-year-old woman with biopsy-proved hepatocellular carcinoma underwent Tc-99m RBC liver SPECT. Findings of this test showed a hot area of blood pooling in the inferior portion of the right lobe of the liver, which is characteristic of a cavernous hemangioma. These findings indicate that positive blood pooling of focal hepatic masses on Tc-99m RBC scintigraphy do not always indicate hemangioma, but rather that hepatocellular carcinoma, which is admittedly rare, may be present.
