Benign Upbeat Nystagmus in a Healthy Neonate

A 5-day-old boy with a history of abnormal ocular movements was admitted. He was born at term by vaginal delivery. From birth, he showed rhythmic vertical oscillations of both eyes for 20-30 seconds, independent of gaze and head rotation (Video 1). Unlike the usual nystagmus pattern, the upbeat nystagmus began abruptly and persisted even while lying still without any trigger points. The neurologic examination was not notable. Laboratory tests, including electrolytes and metabolic screening, were unremarkable. Brian MRI and electroencephalography were also unremarkable. His symptoms were disappeared completely at 5-month follow-up visits. The K-Bayley-III developmental assessment at age 12 months showed normal development.
