Abstract

In practical situations, the root cause may go far deeper down to the bottom level of four or more layers of hierarchical failure causes. The conventional FMEA does not provide a proper approach to deal with such situation. This paper aims to system- atically identify root causes of failure modes and scientifically evaluate the relevant risk priorities for hierarchical failure causes. We proposed a modified method for identifying root causes and a new metric for risk evaluation.
